Coalition Member Survey

One member from each of your coalition's represented sectors should complete this survey. If your coalition does not have representation of all 12 sectors, have at least one from each sector that is represented.

Upon completion, a cumulative average of your coalition member's responses will be provided to your coalition representative via this website for use in the 2009 Coalition Strategic Prevention Framework Survey.
